There are two ways - either have a look on your police force website and put in a search for subject access...some forces allow you to print the forms off. the second way is to go into your local police station and ask for the subject access forms. Police checks are required for those >16years old and cost £10 per person. When the forms are handed in...different areas have different requirements, some request 2 others 1 with photo id for example a passport. |
